MAKING A GIFT TO GLHF THROUGH YOUR WORKPLACE GIVING CAMPAIGN

 Easy as 1-2-3 with simple payroll deduction!

To contribute to GLHF through your workplace’s United Way Campaign*, you simply need to find out who
the employer campaign coordinator is for your company and follow the steps below.

 1. Ask for a United Way Campaign form that includes Donor Choice options listed right on the back of the form. Look for Great Lakes Hemophilia Foundation listed under Community Health Charities of Wisconsin, and make note of GLHF’s unique 5-digit organization number.

 2. After completing sections 1 & 2 with your personal and gift information (be sure to include your signature and the date in section 2), go to section 4, “My Gift Through United Way.” Here you will enter Great Lakes Hemophilia Foundation’s unique 5-digit organization number, organization name, and address (638 N. 18^th Street, Suite 108, Milwaukee). Then fill in the gift amount you want to designate to GLHF, and the subtotal of your Donor Choice gift.   We hope you DO NOT check the box that says “Please do not release my name to the agencies I designate.” We want to be able to acknowledge you for your gift.

 3. Total your gift in section 5, the Grand Total section, and turn your completed form in to your employer campaign coordinator.

 
*Since forms differ among workplace giving campaigns, your process may vary a bit, but you should be able to find GLHF in any campaign that allows Donor Choice or Community Health Charities designations.
